The Rotary Health Check has been developed in Survey Monkey. Each club has its own survey. Click on the link and complete the survey. Please note you can share this with your club members. 

Once the Survey has been completed send me an email to jcollareda@gmail.com and I will send you a report. All reports will be for you unless you choose to share with your Assistant Governor or others.
